I am trying to remove the roll in this guys shirt between the buttons and his left lapel. I'm not really sure where to start. I have tried some stuff with the patch tool as well as the spot healer but I am just looking for some suggestions of where to start.

you could use a 50% grey layer with mode "linear light"
and dodge where the shirt is dark and burn where the shirt is lighter
this could give a different color , therefor you could use a color layer on top
This is very nicely done @colleague. I love the technique.
How did you so accurately select the shadow part of the crease on either side with Dodge tool and the highlighted part of the crease with Burn tool?

I did not use a selection ,
i used dodge and burntool with opacity of brush set to 5 or 10 % and sometimes less
this way you can build it up very slow

So you add a new layer, set to linear light, and select fill with linear light color which is 50% grey by default. Then burn and dodge to get rid of the roll, then color mask it?
