I'm trying to apply a stroke 4 pixels and this keeps occurring, i've used the stroke fx before with no problem until now, I've looked at all the settings and can't find where the problem is. Help please. Dave

Sorry Dave, what do you refer to by 'this' when you say 'this keeps occurring'?

I see a 4 colour restricted man with pen tool selection around him. Now at this stage have you applied the stroke already?
Important question is, while you apply stroke, what do you have selected for your brush tool? Do you have any line strokes applied?
 
This usually happens when there are leftover remnant pieces of background from a poorly made selection of the male subject. Those small pieces get stroked as well and give off the small dot appearance you see around the subject.

Turn off the stroke, place the subject on either a white or black background layer and you will see the remnants. I would use a layer mask and clean up the edges.
 
I thought the image is a screenshot of the Anchor points of Pen tool, as they are all around the image including between arms and body.
 
Just as an example, I made an intentional poor selection of the same image. With a black BG and a white stroke, you can see the remnants of the original BG. They appear when the stroke is added.
