Remove filmy/noise on photo

I need suggestions on techniques to remove the filmy/noise in the photo that obscures detail in the clothes and the overall sharpness in the photo. Also suggestions on how to bring out the faces in more detail. I've tried several things -- converted the photo to B&W, tried working with one channel, reduce noise filter, etc, but didn't get very good results.

Not a lot I can do. With using Camera Raw Filter and Dust and scrathes filter I only got this far:

One thing I would be tempted to try is back scanning of the image. To be a candidate, if you shine a strong flashlight though the back of the picture can you see any better detail?
If yes, with VueScxan or Silverfast SW and the right scanner you can treat the image as a negative while scanning (i.e. as a transparency) and set the scanning software controls for a very long exposure for the CCD to pick up.
If may or may not work yet have had great luck on images that had silvering yet could bring out a ton of detail by back scanning.
Just some quick thoughts to consider.
