Hello! does somebody know how this file description was added?

It's the name of the active layer...

It's the file name with the active layer name in parentheses.....

For example, I changed the layer name to above:

As we used to say in the pressrom, the only stupid question is the one that's never asked.
We're so taken by the complexity of Photoshop that sometimes the simplest explanation runs away from us.
