PDF and two pages

Hi, I hav desigend a form in photoshop and saved it as a PDF. The trouble is it should be two pages. HOw can I get the form to save as two page PDF file. Rather than the one page it is defaulting to?
 
Hi, wood1e,
I'd add the second page to PDF in Acrobat (Document - Insert Pages)... You will need Acrobat Professional, not Reader, for this.
