This seems to be an obvious Photoshop concoction, but it's nevertheless a pretty good effect. I guess you could photograph someone flipping their hair like that and then separately photograph a water spash of that general shape. But putting it all together isn't easy. To me, the part that gives it away is the left side of the spash that contains a lot of dense, greenish-gray areas that probably should be more transparent.
