How to work with album and book covers?

Hello! I hope someone can help me. I have deeply questions about working with photo manipulations as the creation of album and book covers: How does buying images work? They seem pretty expensive on the stock websites, or should my price be very (over)expensive, but even if it was, I would receive less, cause 2 images can be $199. And there is the font situation too. I'm really confused, I want to know how does a person who works with that does, as a freelancer in this case. The stock websites ask you how many copies, I'm afraid I can't tell since I don't know how many the author or musician is up to, what if they get very famous? Or at least how do you do?
