How to smooth real grainy grayscale image?

I'm new to this. I had to scan a grayscale image up 1200% and now it is very grainy looking. How do I go about making it smoother and still hold some of the details.

Here's an example of image I was referring to.

Uded Dust&Scratches filter, then Define from Nik Collection and a little luminance noise removal from CameraRaw. also a little blur of background
In situations like this, when noise doesn't create pattern you will almost always loose details.

@revnart beat me to it but if you don't use the NIK filters (free) I used the camera raw filter to remove most of the grain.
The remaining 'spots' can be removed by using the spot healing tool.

Is this NIK filter and camera raw in lightroom only? I only have photoshop CS5.

CameraRaw is built in Photoshop plugin - Go to Filters > CameraRaw ( Not sure about CS5 - that was some time ago )
Nik filters - https://nikcollection.dxo.com/ - now I see are bought by DXO and stopped to being free but there is 30-day trial ( Again not sure about CS5 support ;/ )
