How to smooth real grainy grayscale image?

I'm new to this. I had to scan a grayscale image up 1200% and now it is very grainy looking. How do I go about making it smoother and still hold some of the details.
Thank you in advance for your suggestions.
 
I would be easier for our members to see the image in order to assess how to tackle the issue since there are many ways to do this.
 
Uded Dust&Scratches filter, then Define from Nik Collection and a little luminance noise removal from CameraRaw. also a little blur of background ;)
In situations like this, when noise doesn't create pattern you will almost always loose details.

CameraRaw is built in Photoshop plugin - Go to Filters > CameraRaw ( Not sure about CS5 - that was some time ago )
Nik filters - https://nikcollection.dxo.com/ - now I see are bought by DXO and stopped to being free but there is 30-day trial ( Again not sure about CS5 support ;/ )
