I need your help for the following:
I have a video of myself performing a dunk and wanted to make a photo out of it which looks like this one:

Anyone know how to do this?

This request needs details. I have no idea what if any video/photo editing knowledge you have. What program(s) you would plan on using to do it, or what knowledge you have of that program.
I or another member could rattle off a how to but if you have no idea what we are talking about, it's not going to do much good. :)

I do have photoshop and don't really have a lot of video/photo editing knowledge but I do know a bit about it and usually i learn fast

Did you understand what my request was or do i need to explain it more precisely concerning the photo?
 
No problem. Just needed to know where you were at on knowledge.
OK here's how I would do it. I say I because there are many different ways to do it I'm sure.
I'm going to assume this video is on a computer and that said computer has windows movie player or a similar program.
The easiest way I know would be to:
1. open your video
2. find and pause a shot of you in a similar position to the first position(farthest from the basket) in the example photo. This will be your main Photoshop layer.
3. With the video paused(and full screen) hit the print screen button(Prt Scr) at the top of your keyboard next to the scroll lock.
4. Open Photoshop, and after it loads completely, at the top far left of your screen is the file menu. Click file/new
5. An option box should open with the word clipboard in the preset drop down menu. click OK and a new blank canvas should open.
6. Go to the Edit menu at the top of your screen(next to the file menu) click on it, go down to the paste command and click that.
7. You should now have the image of your desktop on the Photoshop canvas in front of you.
8. Save and name the file to where you can find it again.

I'll call that part one. Get that completed and we can move on. You can also let me know if you have any questions.
 
We understand the request, but it seems unless you can find enough clear frames of footage within your dunk, using the video editing software you have or haven't got, there is not much we can help you with. Once you have extracted the frames of you during different parts of the dunk, we can explain how to put something like this together.

I hope it was videoed in HD, because you will struggle to find decent frames.

I'll check this out tomorrow because right now I'm not on my computer with Photoshop and I'll let you know as soon as I'm done with this.

I'm on Mac btw if that makes any difference
 
Not really! Unless you had somebody using a camera with super fast shutters and capable of taking loads of pics quickly, you will never get the same results.
It all boils down to how clear the frame are. To give you an idea of how the frames are going to look, I did this using a YouTube video about the first game to be called football.

your not going to get pictures as clear as that from a video unless like others have said you have a video camera with fast shutter speed.
that image would have been done using a still camera shooting multiple images at a fast shutter speed then the guy copied out of each shot and pasted into a single shot

I used a GoPro HD with 60 fps and 720p
 
You need at least 1080p with 240fps to get clear frames as the one you showed in the sample, otherwise is going to look blurry
