How is this Photoshop effect achieved?

Does anyone know how to achieve this effect in Photoshop? At first I thought it was airbrushed but it looks like they did something else to it.

just an off camera flash with a slight smoothing filter which can be a topaz or any smoothen effect plugin
 
Is there anyway to get this effect without an additional plugin? Does Photoshop have any built in tools to help?
 
you could duplicate the layer add some gaussian blur and mask it in to specific areas making sure you keep features and lines sharp then possibly flatten the image use filter > sharpen > unsharp mask tweak it followed by your preferred method for dodge and burning to bring in the highlights and shadows again. or just go to topaz.com and download all there plug ins for demo modes and see if it is worth the investment or not
