How i can create a different thumbnail than actual picture?

Hi folks, i have a strange picture, you can see 2 person on thumbnail of it but on actual picture just a sexy girl xD.

Save it in your computer and see first in thumbnail form then double click on it and see the actual photo.

I am no guru, but that is weird.

How it appears in my folder as an icon.

What it looks like when opened.

Maybe the program used to cut the image, somehow retained alot of the original information? Dunno
