How do you achieve this kind of smoke in Photoshop?

We get a lot of how do I do this, or how do I duplicate that effect questions.
If only it were that easy.
Once in a while there's a simple answer.

Most of the time it's not so simple, it's actually very complicated.

The images you are trying to duplicate are usually the work of very talented artists and the adjustments are usually very, very complex.
There is no 1 button answer or simple A, B, and C solutions.

My suggestion is to post an example of a photo you want to alter, and post an example of the artist work you want to duplicate.
You can post images here on your first post.

That way we can "try" to figure it out together.

looks like a cloud to me, copied and pasted to the girl image then the cloud was smudged over to her lips

I have to agree, this was a pretty cut and dry deal. Someone found some clouds, cut them out, touched up the edges and then manipulated the smoke down to the girl.. It's very obvious it was shopped..

Is this easy to do?
Can someone please explain thoroughly the procedure to doing this?
It sounds complicated to me because I'm not experienced with Photoshop.
Here's another picture similar to the one I posted.

If you take a close look at the smoke you will see a number of areas have just been cloned and the patterns of the smoke are the same
