How do I do this shadow effect?

I've been trying to figure out how to do this effect for a long while & I still cant wrap my brain around it. Notice how the light & shadows creep around the picture? I love this effect! Can anyone help me out? Please?

Image > Adjustments > Desaturate
Image > Adjustments > Levels
Try 170 | 4 | 255 for the input levels.

But play around with the levels until you get the effect as you want it.
