How do I create this effect?

Go to Illustrator, make a path describing the curvature of the ribbon. Use Path -> Offset path.. with some small offset value. Copy and paste to Photoshop as a shape.
In Photoshop: 3D->Repousse->Layer Mask. That's gonna make 3D ribbon... Open Extrusion texture and fill the upper half of the texture doc with anything you want(horizontal gradient in your case)
Rotate and position the ribbon, set lights, gloss, shine, etc...
