Help Please Photoshop is undoing everything i attempt to do! extreme issue!

Everything I attempt to do, will work as long as I hold down the tool.
Then the second I click, everything I just did disappears. I have NO clue what the problem is. Also, almost every time I try to open an image, it refuses to load it.

I have Photoshop CS2. I used it earlier today, and everything was fine!

Please help me solve this!
