Help in designing the perspective text box and 3d popup

Hi,
I just wanted to get an idea how they created this text box effect in a perspective and 3d pop up seen in the bellow attached image if anybody can help me with this would be great help

What is your Ps skill level? This can be important as to how we might need to explain! The more you know, the less we need to go into details.

I have worked for around 3 years in photoshop but finding it difficult to achieve the perspective
 
Perspective is simple..........just create your rectangle and use Free Transform > Perspective.

Free Transform Cmd/Cntrl + T > Right click and choose Perspective

3D as it pertains to your example is about duplicating layers, reducing the fill to 0%, and adding a stroke via layer style in different colors. Then move them using the Move Tool.

You can also add blur.....

If you want a similar effect to the sample image............you will have to be very good with the Pen Tool. But that would be very difficult to explain!!
