Hi!

I want to create a picture like this: Google Bilder-resultat for http://farm4.static.flickr.com/3409/3256194469_8549469b11.jpg

I know I have to take a picture of a hand and five heads, but I'm struggling to make the transition from the neck to the fingers look natural. I have experience with Photoshop, but I'm not quite sure how to do this.

To make the fingers and the head have the same color, I will try hue/saturation, contrast and I will try to shoot the pictures with the same light.

What tools should I use to make the transition look natural? Puppet warp, smudging etc..?

You need to understand how to use masks and how to match colors.
I don't see how puppet warp works here

It looks to me like all of the separate images were planed out with this in mind.
With the right images this is very easy
 
Steve's right. Puppet warp won't work here. You have to take pixes to match the angles of the finger tips. Take it in medium lighting.

You can use brightness/contrast to match the skin colors.... not necessarily the hue & saturation. Sometimes it takes a bit of imagination to merge the joints with the clone tool in airbrush mode with a medium flow.

You can also use the lighting effect plug-in.
 
To match the skin colors I usually use simple technique: Select head, hide the selection. Now go to each channel in turn and use Levels. Find the Levels controls positions(try all of controls, don't forget the Output levels) which get you the seamless and natural transition between the head and finger. And so on for each channel.

This is very powerfull technique for digital transplantations of various body parts. It helped me a lot in the past. Of cause, chances are you'll need some clean up work afterwards: Dodge and Burn to correct the shadows, History Brush or masks to revert some points, Hue/Saturation - in case some colors get washed out...
