Choppy brush tool on CS5

I'm using a Cintiq 21UX tablet on my newly installed PhotoShop CS5 and, despite how much I play with the brush settings, the lines always come out jagged.

I tried updating my graphics card, removed and reinstalled my tablet drivers, updated my tablet, and even tried drawing with a mouse and a bamboo to see if it was just my cintiq at fault. The problem still persists and its starting to become a hassle.

I'm starting to pull hair out over this. Any help would be greatly appreciated.
 
Maybe I didn't explain it well enough in my first post but after I had the problem with my Cintiq I tried using a plug-in mouse and then another wacom tablet. The brush was still jagged for all three of them, even the non-wacom computer mouse.
 
The brush settings should be at their default right now. I did play around with the settings then I did the ctrl + alt + shift reset. I haven't touched any settings since then.

Though I think I made some slight headway into my problem. The brush shows up smoothly if I select 'Actual Pixels' but horribly jagged from any other view. I don't know if I'm just imagining things but the zoom on actual pixels in CS5 seems a lot closer than it did for me in CS4. Its very hard to work on everything at that distance so I'm really hoping there is a way around this.
 
Have you tried using the tablet for other painting programs, like MS-Paint or GIMP? It might not be Photoshop at all, it could just be your computer has odd settings, malfunctioning drivers, incompatibilities, or a malfunctioning system device controller.

Haha the method I use to draw if I'm going into explicit detail is an Xbox Controller... I ordered one from China (lol it was $20 new) and it had the same problem with everything, even games... Big fail :D It only moved on 4 axis, no diagonals or anything (it was a microsoft fraud controller, damn chinese!)
I got a new one though and I'm working with it just fine now tho.

Yeah it was the same thing with the mouse. I feel stupid now but I think I fixed it. Not sure how but installing an older graphics driver seemed to fix it. If I update the driver then the brush in CS5 goes right back to being jagged. :/
 
...Then I restart the computer and the brush problem comes right back. And it does seem to happen in paint too so I suppose it isn't a PhotoShop problem just my computer falling apart or something. I guess I just assumed it was PhotoShop because the problem didn't start until I upgraded to CS5.
