Can someone repair/clean up this picture?

Hello. I'd like the following done if possible. This picture was cut into a heart shape but I would like to salvage it. Let me know if $5 USD Paypal is ok.

1) Remove the few white spots from hair
2) Fix that cut up part right above her head
3) Sharpen the picture a bit. Seems slightly blurry
4) Extend the background or add color to the bottom left and right and other areas needed so it can be a normal 4x4 or 3x5 size type picture
5) Clean up that curled part on the bottom left with the light reflection
