create a new document
use the shape tool to draw out your box with the color you want it to be.
then on a new layer use the pen tool or the eliptical shape tool and fill with white so you get the lighter shape over the box
In the layers panel right click the white shape tool and select create clipping mask so only the part of the shape you want will be there wherever the underlying layer is.
Set that layer blending mode to soft light.
create a new layer with the white dot on using either a hard brush or shape tool, right click that layer and select blending options to add a drop shadow.
